We are asking all parents and players to submit feedback on their Head Coach for the 2019-2020 hockey season.

We've put together two online evaluation forms; one for the player and one for the parent to complete.  The evaluation is anonymous.

Please take this opportunity provide feedback to the SSMHA Management and Executive board in an effort to continuously improve the experience for all of our members.

We will be accepting responses until May 31, 2020.
